What happened with me was that my xbox had stopped reading games, i opened it up and realised that my lense was trying to read the plastic edge of my game as the lense was positioned to far back for it to read my games.
To fix this i simply wrapped some tin foil around the bar in the picture, this allowed the lense to not be so far back which made the lense read my game instead of the edge of it.

you should see above the laser (2) gold pin holes the one on the right TURN 90 degrees to the right that should tighten the laser...i will make a tut...this shold work...it worked for me havent had a problem for ever now
