Most people don't realize the difference. Someone can be ripped as shit and still be considered obese. The fact is like you said, America has a lot of big people (not fat). Although there are quite a bit of fat people it is not as bad as everyone makes it seem.

It's the people that body build etc that raise the obesity level.

Definition for obesity:
Extremely overweight, especially: weighing more than 20% (for men) or 25% (for women) over their ideal weight determined by height and build; or, having a body mass index over 30kg / m


So as you can see someone can not be fat and still be obese..
