I finally worked out how to obtain this achievement. Work your way through CAREER and win 6 titles. Once you have won 6 titles you get the option to DEFEND A CHAMPIONSHIP. Simply defend any of the titles 3 times and the achievement will unlock. I defended 3 different titles so contrary to other guides it DOES NOT have to be the same belt that you defend 3 times.

A quick way of doing this is to play through till you get the TABLES stipulation. You will also need the ABILITY to use weapons better (grappling with weapons). Once this is available and your stats are 90+ you can win a match in a matter of minutes. Simply leave the rind, pick up the table, hit the opponent with the table a few times to build up your SPECIAL / FINISHER and the finally GRAPPLE with the table to set the opponenty up for a TABLE FINISHER and they will go straight through the table.
