You need to clean your laser inside the disc tray, take the xbox apart, then the disc tray and use a cotton bud or something just to clean the laser.

i had the same problem here are your solutions
1. clear system cache
2. open the tray and use an air compressor inside to blow out any dust
3. your laser is damaged
4. something has gone wrong on your hard drive and you must send it off to microsoft (which is what happened to mine) , on mine a little elastic band kinda thing fell off thats all

You need to take apart your xbox, and clean your laser or get a professional to do it.
