It's pretty simple. The controller was supposed to come with only the Lancer code as Rod, myself, and others have stated. However, a printing error said all 5. Their mistake is your benefit. Because of this, the controller code now includes all 5 weapons. I made the change personally, so I can 100% say for sure that you get all 5 weapon skins.

if you beat gears of war 1 on insane mode, you unlock golden sawed off
golden gnasher is veteran gear exclusive
opening week you unlock gold lancer, and golden hammerburst.
