Its just like a certain aread you spawn that may have huge floating mountains or giant water falls. Or sometimes huge caves with ores in them. I think its just more to do with the area of the world you spawn in nothing else changes, just the area around you.

Pretty much like what the world is like, Some are just Flat land things and some are Huge Mountains, Cave's, Dungeons, Etc.
