So my friend has MW3 and he told me a few things from campaign here is proof he has played/playing MW3 and has completed MW3:

+ So from what he told me he says that Ghost is still dead he doesn't come back.
+ Soap is killed by Makarov's men
+ In the ending Captain Price beats up Makarov and jumps on him then hangs him *epic scene*

Hope this cleared a few things up.
