A small island that you become jarl over and get a castle to rule from. You can have a privately funded army that advances in its numbers, weapons and strength as you upgrade it by buying the upgrades from your assistant. The assistant could also advice you and manage things while you are gone. You could also have your own court mage and an assassin. The assassin would kill any unique character in the game that you tell them to or die trying. The castle would also have a moat and draw bridge. You could have missions to find rebels and people committing treason and hang them from gallows. Also you should have a lot of missions and story line in order to become jarl in the first place and then when you do you should receive unique jarl robes.

A massive addition to wearable items and a possible sun bleached dragon skull to wear on the back. It could be an extremely rare drop off of certain types of dragons, with the chance going up the higher the difficulty.
Also a more thorough background on the Thalmor and more homes or home add-on purchases.
