You unlock via Elite clan and your clan has to enable it as your clan emblem and then you equip the clan emblem and you got it on MW3.

I know how to get them. These emblems are obtained through call of duty Elite clan. At the moment they are currently unavailable as your clan need to take place in the Call of Duty Elite Clan challenges with aren't currently up and running (Hoping they are up and running soon) P.S There are also titles similar to these.

There unlocked through elite when you level up your clan it tells this on the elite website

You'll unlock them once clan operations are released, also maybe by participating in competitions/matches.

You get these in elite, when your clan completes a challenge. Example, "Capture 30 flags in domination"
stuff like that.
