that used to happen to me and i tipped my xbox back while reading the disc a few times and it was fixed. did you happen to be jumping around alot while the disc was in the tray. you might have a circle scratch.

ring up microsoft and send it to them, they will replace the cd drive with a new one.

i found a way it worked with me but it deepends how deep the scratch is follow these steps :-:
1.Get your scratched disk facing up (so the scratch is facing up)
2. Get a tub of toothpaste and run the tooth paste over every part of the the disk (make sure the toothpaste is only a thin layer but not to thin)
3.Leave for about 5 minutes
4.Now put water over your fingers and slowly rub the toothpate of gently .
5.Now leave to dry for another minute
NOTE : Any other water just rub gently of
