Every so often this will happen to me and it gets quite annoying after a while? Does this happen to anyone else and how can I fix this?

If it is a server, just back out and re-join.

Single player - exit and go back into world.

It most likely is a rendering issue. Try installing the Optifine mod.

Its rendering slow. You may have too many mods slowing the game down.
Download optimine and it should help out a ton.

It most likely is a rendering issue. Try installing the Optifine mod.

I had the multi-processor optifine when the problem begun so I swapped to smooth but still the same problem was there. I even tried removing optifine but this problem still occurs. Any other mods which could solve this issue?
