So I was out on a date tonight at a pretty expensive Mexican place with great food and I got there and we both ordered our meal and I ordered a shrimp and a steak taco plate with no cheese but when it came out from the kitchen there was cheese on it. I very politely pointed it out (I can't have dairy as I'm lactose intolerant) and she hurried back to the kitchen and brought a new platter out with no cheese and everything was fine.

But when I got the check I noticed my meal (supposed to be 15.99 or something like that) was 8 bucks and she said she made it half off for the wait. Well the total was 25 ish with my dates food and drinks and I felt bad for hassling the waitress so I left a 30 dollar tip and left.

Does anyone else feel really bad when stuff like that happens? For example your order gets messed up or someone providing a service screws something up and it's not a big deal? I always feel guilty about it...

Yes.

Whenever it's not their fault, I always feel bad because they lose tips in situations as described.
More often than not I tip the waitress twice as much as my meal cost, unless the service is really bad.

I don't feel bad. I would tip the waiter the same amount depending on the starting price. When you ask for the food a certain way, and they agree to it, it's their JOB to make it that way. When they don't, I actually find it rude. It wasn't the waiters fault though, so I would make the tip the same.

I always feel bad, but I would sure as hell not leave them a $30 tip.
