Have you ever been banned on this server before: Nope, Iv'e never been banned from any server, aswell.
How long have you been on the server: Im about to join, but I will be on this server everyday, or when I have free time, which is most of the time.
How many times have you been a mod/admin on another server: Im currently ADMIN, on Pheniox Craft, but it's been down forever, and I might leave once, it comes back up.
