Maybe its like psychological pain, for example if someone tickles you, before they actually do, you start giggling because you know it's going to tickle, if you do it to yourself it won't tickle because it's pyschological. It's hard to explain but maybe because you know pinches actually do hurt, your mind made the pinch in the dream hurt.

It was actual pain I felt though. I pinched myself more than once just to make sure and everytime I pinched myself I felt it.
