If your on xbox go to the dashboard then go from guide to system, system settings , memory then press Y then clear chase and you will have them when you start up mw3

Try clear all your storage devices then restart your xbox!?

That cant be it cos im having the same issue and ive had premium since the game came out with the harderned edition
also my spec ops rank has returned to 1 and ive lost all progression in spec ops
