Guys, I hate to break it to you, but it isn't hard to get. I had it yesterday, by playing multi-team with 6 perks, no guns, and a stun.

200 kills, 10 backstabber medals, 5 survivor medals, 5 revenge medals, 5 bloodthirsty medals, and kill 5 enemies with their own weapon after killing them with the combat knife, that's all you have to get.
