there should be the codes in your game case on the back of the paper, just redeem the codes and they should be there

The camos I think come with the maps. I'm not sure. I haven't redeemed mine yet.

The code for the camo and playercard are on one of the coins that you get. Then you enter it at callofduty.com/blackops2ce
