Odd, I had a similar issue with the campaign, on completing it on legendary, perhaps one of the chapters you've completed didn't register, you may have to review all of your completed missions to ensure that you've completed them. Are you sure you've completed the newly released episode? Perhaps you read the achievement incorrectly, as it reads "Complete all chapters in the first 5 episodes of Spartan Ops on any difficulty."

You've read the achievement wrong, the achievement reads "Complete all chapters in the first 5 episodes of Spartan Ops on any difficulty." A chapter is an individual mission, an episode is the set of missions you must complete. You'll need to complete the newly released episode, episode 5 in order to obtain the achievement.

You are very correct, but a lot of people has had this problem even though they did everything that's required, so don't worry you're not alone.

I had the same problem with the challenge achievement.

What I had to do is get a 26th challenge to unlock it.

We may have to wait till episode 6 to unlock it

got the achievement now after playing the 1st chapter of ep 5 again with a friend. 1000gs in halo 4 now

343 sucks! Everything about halo 4 is broken I would have rather had the story end at halo 3 than have to play a horrible broken game made by 343.

Complete Chapter 1- Ep. 5 on Easy SOLO! It's a glitch on waypoint. If you check waypoint it would say 20/25 SOLO completed. That's why you don't get the achievement.
