I modded the Focus aromr and the Locus helmet 3 months a go and I also play modded gametypes online on Halo 4 and I have not got banned yet.
But on the other hand on an other xbox account I modded the Headless mod (No Head)! and that account got banned from xbox live (permanently)! after 4 months.
At the end of the day: "If you mod, there is always a chance of getting banned."

There is always a risk but ive done mutiple accounts in a matter of months and i have not once heard a report of bans.

As almost in every game, there is a slight chance of getting banned.
