Because you are not closing the disc trey at the correct timing. I use to get this. Manually open your disc trey keep it open put the game in there do NOT close it, click copy file and it will close the disc trey for you.

No problem. If this do not work then i would advise you to buy a need disc.

i thought it was the disc cuz of some scratches but i but i got the disc repaired and it still did the same thing.

Yes it is the disc then, just download the game from xbox 360 iso and just manually move it on your hard drive through Xbox 360 Neighborhood or Freehood360.

i think its my disc reader cuz its doing this for every game.
