Yes there is a good few, here is a post with official hosters Forums/t=5480669/a-list-of-legit-...lease.html Their posts are normally flaming so check them out and hope you get in one, I need one too, no luck though!

Yes its like a 90 to 10(Percent Wise) the 90 being Fakes/Scammers and the 10 being Legit hosters

Yes there are, but not everyone wants to pay 50$ a day for XBLS to go online.
ALso, XP lobbies are still free and easy though.
