With mods i used to use, you could only do it online in zombies, and you needed an extra controller, i never got banned, but be careful, theres always a risk.

To clear things up:
*USB mods can only be used on solo or public games, not private match - as the Dvar enableDvarWhitelist is rest along with other infections when entering private match.

*Online jtag/rgh mods using a modded xex can host private match, as the mods are not infections to bypass the patch. However with this, you will probably be banned very quickly.
