Yes, I do that with all of my games, but it's still coming up as unreadable.

And if you have a steel case like the one my hardened edition comes in, it can happen very easily. the same thing happened to my MW3 disk. I completely agree with you. There has to be a different way to distribute the games.

just buy the game used from gamestop then return it with the old disc of GTA in there

This has never happened to me but yeah, I always thought that could happen but never thought it actually did.

Here is what I have been told about taking the disk out properly.

You have to place the box onto a flat surface, and then take out the game, because if you try to do it while your holding the case in your hands, you can snap your disk.

The same thing happened with my MW2, it was pretty annoying and got my quite frustrated.

I agree that they should redesign their case, it would save a lot of people the hassle of having to buy a new game when it gets broken due to their crappy innovation of a game-case that does more damage then good.

That's why i put mine in a disc sleeve binder.

I think you don't just rip the disk out. Don't you press the button in the middle and the disc pops free?
