I recently contacted Xbox Support because I was supposed to receive a redeem code for Killer Instinct for having a high Xbox tenure on one of my accounts. Well, I received the code, but was unable to redeem it because I only received 24 characters, and prepaid codes are to have 25 alphanumeric characters. So I contacted Xbox Support and gave them a brief explanation of the problem. They asked for a few minor account details and then told me that they are unable to issue codes for Killer Instinct. I told them thank you for taking the time to hear my case, and the support representative said, "But I can offer you Dead Rising 3, Ryse: Son of Rome, Zoo Tycoon, or Forza 5 in place of the Killer Instinct."
I am happy to tell you that I took Forza 5.
I am not telling you to go out and try to get codes, but to those that have been having issues with their Killer Instinct code, or even getting one, then you might be able to get a replacement game instead.
Now, many of you would like to be given proof to be certain that this did take place. So proof is provided below

Thats cool, but killer instinct is a free to play game and doesn't require a download code.

Correct, but they were giving certain gamers (5+ year tenure) access to the Ultimate Edition for free, and I was one of the lucky ones
