A KeyVault (KV) is a file on all Xbox 360 consoles used to communicate to XBL servers and servers communicate back to the console telling it if it is banned from Live or not.
you can't 'make' a KV. The only way to get one is to get an old console that isn't banned from Xbox Live and extract it straight from the console, and you need tools like a soldering iron and a NAND-X or JR-Programmer.
