I would put Modric in CAM and Fabregas in CDM

Never put someone with H/M work rates as an only CDM, ignore him xD
That's actually a nice team but your defence are way too attacking. Having a high attacking work rate on all of your back four will cause you trouble. I would recommend getting Puyol, Inigo Martinez, Pepe, Miranda etc.. to replace Ramos and Pique. Maybe Jordi Alba for Marcelo? From personal experience, I liked Dos Santos more than Greizmann. Also one last suggestion, Song in CDM. It may be a cheaper team, but trust me you will win more games.

Yeah, work rates are as important as chemistry. If your back line all have H/M or H/L, chances are when you take a corner and your opponent clears the ball, he will be through on goal with all of your defence in his box. I always recommend a high work rate on a LB/RB, M/M or M/H on CBs, M/H or L/H on CDMs, H/H on RMs and LMs, M/M on CMs, H/M on CAMs, H/M or H/L on LW, RW, LF, RW, CF or ST.
If you use these correct work rates, you'll see the improvement in not just your squad, but your winning ways.
