Intro: For my intro I would like just a simple intro with moving shapes and what not and my name (FalloutGamingHD)

Outro: Outro I would like just my name at the top (FalloutGamingHD) with a Twitter Button, A youtube button, and a subscribe button along the buttom. Also two Boxes with Question marks in the middle saying "Mystery Video"

Color Scheme: Black, Blue, White (Not to much white)

For the Youtube and twitter buttons, I would like a certain type. I will provide links to the download to both of them.
