My Graphics Card is a AMD Radeon R9 280x !
Whenever I play my screen goes like laggy and choppy almost, just wondering anything I could do to fix it, should I try re-installing it or what should I do ? +REP for the best answer or option. Thanks
Also I took a PC Test on 'Can You Run It' and here are my results.

There's a lot of reasons why it could look choppy.
I'd to know more info before I can properly assist you. ie: Current refresh rate, are your drivers up to date, are you playing Day Z etc.

I play DayZ Epoch and I was Playing Arma 2 OA and I get the same results on both, and i'm still pretty new to PC's but I went to Control Panel then my Arma 2 OA folder and it said my refresh rate is 60, i've seen it at 75 before.

I would say maybe clear a few unwanted things off your PC which may be slowing the process, causing the lag
