Coins come into the game from 2 ways.

Games are played / tournaments / etc granting you coins.

Fifa Points are purchased and packs are opened...

These are the only two ways coins actually enter the game.

There used to be a glitch (which is now patched) that enabled you to gain an unlimited amount of match earnings profit in a short period of time. People abused this and flooded the market with excess coins. (Primarily for coin selling)

If you want to get technical with it, another way coins were introduced was the consumable glitch as you could quicksell the duplicated card and bring in MORE coins into the game (auctioning it off means that existing coins were just shifted around)

No, they're all fake. I don't even know how you would make one. You would have to steal someones code and enter it in the time it takes to check out the card (to activate it) before they get home and use it.
