Keep the top button and the bottom pressed until you seen the apple logo once seen that release top bottom button. This will enter safe mode.

Well when its tethered and you reboot it won't load safemode Cydia Substrate as you'll need to run your jailbreaking application again.

Though what you could do is run your jailbreaking application to get into jailbreak mode again but as it is loading up hold down Volume UP button. So this should boot into jailbreak mode + safemode in case you installed a mod that doesn't work. I haven't tried this myself but I'm assuming it works :S
