Just dont turn on your computer or xbox or playstation. And go study. Or you could ask your parents to keep your console so you cant play.

You could tell you parents when to turn you console or pc off...
Or you could study then play games after your done.

Yeah I'm in the same position I'm failing 1 class (25 average) the rest I'm barely passing. Finals are coming up and I'm going to bomb them I'm 15 also.
Basically heres what I did for about a month or two and it worked but I gave up because gaming took over.
As SOON as you get home do 1 hour of studying/homework after a few weeks bump it up to about 2 hours if you think you need that much time. Promise yourself you won't game until all your work/studying is finished.

If you're using an Xbox 360, use the 'screen time' feature, it allows you to cap the number of hours you play, it prevents you doing anything once you reach them hours.

Get your parents to set up a parental thing on your computer/console so after like 4 hours or at 6pm, it turns off and you can't get it back on, then use the rest of the night to revise.

You could try studying with a friend on Skype or something.
