I don't know that there is a definite one that is the scariest.

Some scary ones are:
The Blaire Witch Project
Paranormal Activity (all of them)
The Omen
Sinister

The Conjuring was rated the "Scariest Movie Ever Made" last year, however. I thought the movie was kind of funny and not scary.

I think that the human centipede is definetly the most messed up one I've seen

Well a lot of the movies made today aren't that scary, but a recent one I thought was pretty crazy was definetly Sinister

Insideous one was the scariest film I have seen the second film it only made me scream once and sinister is also pretty scary
