Who creates them and gives them to people?

Well, to be technical, nobody actually "makes" a KV. The KV that is in your console basically determines whether or not your xbox is banned or unbanned. That brings the names for the KVs. All consoles even a retail right out of the store brand new consoles. Have a KV. So the "makers" basically just extract them from these unbanned retail consoles and sell them.

Oh okay but what happens now because im sure they have stoped making xbox360 consoles?

MS has not stopped making consoles and won't unless they don't draw in revenue.
There's plenty of consoles, which means plenty of KVs. XBL will stop being supported before KVs run out.
Even if they stopped making consoles think about how many are in circulation, and think about how few people there are that have RGH/JTAGs to the people who have retails.
