If you decide to replace your thermal paste then do it as the guy did in GT-R's video, but just put a pea size dot in the middle and don't spread it out. It will spread out on it's own through pressure and heat.

Anyway OP, there is a lot of things it could be and a lot you could try. It might even be your CPU. Try venting the case a bit more, clearing out dust, making sure all fans work. Changing the thermal paste on the GPU may void your warranty but that card is so old the warranty probably isn't valid anymore.
But just download Speccy or HWMonitor to properly diagnose the problem.

Please explain how one would cool a CPU without the use of any thermal compound.
As for the graphics card, check to see if there is a lot of dust build up. If there is, take a can of air duster and clean it out.

OP: As far as I can work out, your GPU is built into the motherboard and is probably cooled by a little heatsink or [maybe] passively... Id check to see.
Since your PC is so old, Id recommend changing out the thermal pads. Yes, pads. Since its probably a small heatsink, the manufactures would have used pads. [again, id check to make sure]
Or, you can get a small dedicated GPU [even a cheapo one] to fix the lag.
Either way, Id get dirty and clean your whole PC. Clean out everything you can and see if anything improves.
[I dont recommend this but its what id do]
When playing WOW, have a feel inside. See what is causing the heat. Once you have that, you can look at replacing whats needed.

WOW. If I could be bothered, I would record my self doing a desk palm but I CBA so google it.
Anyways, Thermal paste is used to transfer heat from the GPU to the heatsink where the fans cool the heatsink cooling the GPU. Fairly simple process which wouldn't work if you dint have the thermal paste to transfer the heat
