Really depends. Modding would be a permanently most of the time, if not it will be 2 weeks than a 9999 ban. When it comes to petty stuff, I don't think anyone knows.

-It's 3 for petite things including -False Reporting etc

-It's permanent for hosting modded lobbies/using mods online(excluding non monitored games) if they believe you have a modified console then you will be console banned

-It's 3 strikes for playing with someone with a modded console purposely includes recoveries for some reason.

I asked a support member at MS and they said it depends on what you do but normally if you get like 3 bans that are like 2 weeks it equals to a permanent ban.
