I just can't seem to figure it out and I've tried for the past hour

Can someone make this where when I click on the image it takes me to a website?

Well I'm looking for it for a different website that requires what I put. I just tried that and it didn't work.

Still won't work for the site I'm on. I need something put into the 2 links I pasted up there. I've had them before I just don't know how to do them...
