I went through the exact same boat man, what you have to do is just realize that it's not your problem or life, if she wants to harm herself over you then let her, don't fall into her traps and just leave her. Trust me. my ex had depression and said she was going to commit suicide when I left her, she never did.

What I'm trying to say here is just to politely tell her to back off and if she tries to pull any sneaky shit with you just block her in all ways or forms.

This is probably the best advice i got thus far man.. thank you +rep for dat.. also did you involve family or anything?

Nah just told her straight up to **** off she was making my life miserable my annoying me and being insanely clingy and kept texting me that she loves me and wants to talk. lol.

Oh shit, I had the exact same thing sort of, ended it like 2-3 weeks ago.. Minus the self harm part but add another 6 months.. Anyway I just ended up not caring anymore, decided she was too strange so I just told her I had stopped caring etc etc and told her I wanted to stop talking/seeing her and just ignored her after I said it.. Seemed childish but like you said, everytime I was ready to call it quits she would blackmail me into staying together with her.
