If your front end is 5" lower than your back end, you either aren't measuring right, or they put a bigger block in the rear / a coupe add-a-leafs in the leaf pack. I'd take the block out if that's what they did.

As far as the front, what front suspension do you have? Coil spring, torsion bar, or leaf spring? I have torsion bar, but I don't think Dodge used torsion bars in '01 so I'd bet you have coil springs. With that being said you could go the cheap way and get some coil spacers (you put a spacer in between the coil and the coil pad), or you spend a little more money and do it the right way which is to get bigger springs. Both will work, the coil spacers just don't ride as nice.

Keep in mind though, when you lift the front end 5", you will need new shocks. If the rear truly is lifted 5" above the front, see if the rear shocks are maxed out or if they got bigger shocks.

EDIT: The rear of your truck should sit about 2" higher than the front of your truck, this is rake. They make trucks like this so when you load down your truck, the truck will sit level so you aren't popping wheelies down the freeway.
