So I was chilling with my girlfriend and I noticed there was a new guy there, (on her contacts) I asked who was he and why was he there. She said " I never noticed him I deleted him a long time ago, I don't know how it came back." I just want to know if there's any type of bug or something to make the contact reappear out of the blue or if I'm getting played

I've been using iPhones for a very long time. This has never happen to me before. My best guess is your girlfriend is lying to you or maybe it is true but I really don't think it's any bug.

Now with Apple, i am not sure. But with android upon getting a new phone and the same SIM ETC Contacts within two months of deletion have came back.
But your situation seems a little more suspect then just "deleted".
