I've tried updating, rolling back, reinstalling drivers. Nothing is working.

This is driving me insane.

NVIDIA GTX 950

AMD 860k OC'd to 4.0hz

8GB DDR3

144hz Acer monitor

Do I dare say it may be the OC??

I would normally get this issue on a bad OC on a GPU.

Is that OC'ed?

Yeah it's very common for unstable OC's on a GPU or CPU to cause issues like this or bad sectors in the RAM which this isn't.

I fixed it. The problem seemed to be the resolution I was playing on (1600x1080).

Well.. can't say that was the issue for sure but I changed my resolution and it fixed it. hmm.
