Try to update using a flash drive.

Make sure you've renamed the file to $$ystemUpdate.

I figured you've tried most this but I believe it's your plugins

Do I need to put it on the Xbox hard drive if so where? Or do I just plug in USB like I have and update then take it out

Just plugin the flash drive let it do the update and reset to make sure it stuck afterwards.

Ok so from the looks of it work!

Am I able to unplug the usb

Yea unplug the usb and do one last reset to make sure it's sticking then go on mw2 and join someone or a game to make sure you don't get a fatal crash.
