If you download a MW2 menu, just as an example say the Aspect Modding menu. It should come with two files in the folder.
One will be dll_load.dll & the other, default_mp.xex
Place those both in your MW2 directory & launch default_mp.xex
Make sure you're on TU8, if your RGH is online it will prompt you to update once you boot MW2, if you're offline you can find the TU8 download online.

If you purchase Destruction v1 you would put the Destruction v1.xex as a plug-in in dashlaunch then save and reboot, once you reboot and it loads the dashboard you should see a notify that says Destruction v1 - Loaded.
Then boot MW2.

Others are .dll files. Go onto root and change it the .dll file (replace it) should be on the menu folder what you will download.. and change the default_mp.xex
