Thinking of getting Nainggolan's OTW but not sure how he's playing atm so I would like some advice if possible.

He has only scored once in almost 6 full matches and there are for sure way better DM's in both Serie A and indeed the other leagues on FIFA.

I would get someone else, or perhaps his standard card as it seems a lot of coins for a small chance of a payoff, though if you can get his OTW and still get a good team around him then go for it.
