it doesnt only happen on launch... it happened to me on villa, and grid as well... when the host leaves than turns their Xbox off and they are a party leader... anyone in their party the spot they were in in the game says they are there, but since they are not... they cannot be killed... therefore, you get hitmarkers but no kill... there is NO way to be in the game and have this happen to you
