Try re dumping the nand a few times, seems as if your nand is damaged. (might be missing blocks)
If that don't work i'm not sure what to say.

Happened to me a few times, im no expert but go to your jtag and boot into xell (turn your jtag on by the eject button) and it will say what your CPU key is and you DVD key.
& are you using the right button, like did ur jtag have the
aud-x clamp fix (think thats the 1 option)
or donor files?
Like i say im not a expert myself but yea just trying to help ;)

Read your nand 2 more times and then compare them with this program.
[ Register or Signin to view external links. ]
If you get a message saying 0 non matching blocks found then you have got two identical nand reads which is good,if not then read yet another nand and keep going until you get two matching nand reads. You should get two identical reads straight away if you are using a usb reader.
Then go from ther and follow this tut, it will make things much much easier :]Web Page Name

When you create your freeB00T image select donor nand.
If you flashed the nand in the past it is a donor nand.
Sorry i couldn't mention anything sooner.
