Mine did the same thing but when I look really, really close I saw a small scratch so I just made a new disk using the same patch and then it worked. But now I have a Jtag so it doesn't matter if I hot swap or not. I don't need to. Hope you figure it out. Best of luck.

Thanks for all your help guys, I will have a look at it tonight and then let you all know. I noticed when I put the iso disc it sounded like my laser was making a weird noise like a sparking/scratch noise it's weird. I tried it with 3 disc and it does it on all of them. I am going to take my laser out tonight give it a good clean and then try again

You May Have Broke Your laser When Taking Your xbox apart and may have broke your harddrive i dont know but try stick a usb in and save it to that
