I had this problem, I'm not sure how I fixed it, but maybe what I did might give you some ideas.
#1 Downloaded GoW3 VIP from Downloads section on TTG (Froze on startup), transfered to Xbox 360 HDD plugged in to computer internally.
#2 Formatted 4gb Flash drive, transfered to there using Modio (Didn't appear on Flash Drive)
#3 Formatted 8GB off my camera for Xbox 360 files (using configure option when formatting), Used the download in the sticked thread, updated with GoW 3 89mb file on flash drive, then transfered update, and original file to HDD = Success.
I'd just take what I tried and play/test until you get it.

i have the vip code i redeemed mine the pro way not usb modding...jk but everytime i play it just straight kicks me off live there may be a update for it idk havent played in 2 hours
