I've seen the same thing happen, just a little differently. My friend shot his upgraded scavenger over by where the guy used the flipper nuke, and George just fell through the map.. Maybe just an explosion near there is what causes it?

Im not sure maybe somthing could be made of this for example the flopper or the ray gun shot.

That is really interesting! I don't know what could have caused it, but let's look at what happened after he fell.
After he fell it sounded like he was saying "oh no". After he disappeared the little girl made the Teddy bear sound. Now it appears that this is part of map, meaning it is supposed to happen and not a glitch.
Did George to away for good or did he come back after a few rounds?

He didn't come back for like 30mins maybe a little longer. and i see it made the sound of for example you got the tedy in the box.
