this could have multiple causes:

Uncleaned hardware, make sure your pc's or consoles hardware isnt dusty, take the case off, and try blowing any dust out of it. If it's too hard try searchng a tutorial on youtube.

Dirty or scratched disc, try installing the game, for xbox only.

Insufficient harddrive space, the console or pc itself needs suffient space to function correctly, try having atleast 10gigs of hardrive space for any console, and for pc, somewhere at 15+ gigs

If this didnt help you might want to replace hardware or the disc

thats your disk man

try cleaning it ..other than that..buy a new disk
