Create a crafting table and scroll across to your decorations category. I believe there's a description stating the materials needed.

A skeleton needs to kill a creeper with a arrow
they can be found in dungeons as well

Skeleton needs to kill creeper just stand behind the creeper but not too close and try to make the skeleton to hit it until it dies and you get the disc

You might find one in a dungeon? Or you can get a skeleton to hit a creeper and kill it which will give you the gold disc. Hope this helps
